Complete an order form with the following information:
Name and email address of person to receive the certificate
Name of generous person giving the certificate
Denomination (in whole dollars) you'd like to give

All proceeds go to the dogs. Certificates are good for one year. Recipients are notified of their Certificates via email, and can access it through the Internet. Certificates can be sent via snail mail by special request.
